This is Dr. Carlin Vickery. I'm a board-certified plastic surgeon with 30 years' experience, practicing in New York, NY. The following video will cover a candidate who has undergone surgery. The photographs will include some nude shots of surgical sites. Reviewing this video will give you an in-depth understanding of the surgical procedure and ideal surgical results.

This patient is a 19-year-old girl who is 5'6" tall, and presents with a 5 year history of severe macromastia. The patient has had large breasts since she went through puberty, and she suffers from chronic neck, shoulder, and back pain, which limit her ability to perform sports, and also causes depressions in her shoulders. The slide demonstrates large breasts with hanging of the breasts below the inframammary fold onto the abdomen, such that they are approximately halfway between the end of the breast on her chest wall and her belly button.

In the next photograph, you have an oblique view, and again, you can see that the attachment of the breast on the chest wall is significantly higher than the base of the breast tissue, causing the ptosis, or hanging of the breast. In addition, the patient has very widened areolas, which is the pigmented portion around the nipple. The breasts are not well-supported on the chest wall, causing her to have a pulling sensation at all times

This is a full lateral view, again demonstrating the hanging of the breasts and the large volume of breast on the abdomen.

In this photograph, you have side-by-side the preoperative and postoperative photographs of a vertical mammoplasty technique, demonstrating that the breast have been made significantly smaller. They now sit higher on the chest wall and the nipple areola complexes have been reduced. If you look closely on the right hand side, you can see a faint scar going around the nipple and down the center of the breast. In most of these patients, this will heal in a very faint fashion and be very acceptable.

In an oblique view, you can also appreciate the decrease in the volume of the breast as well as the narrowing of the tissue away from the armpit and towards the central aspect of the breast.
In a side view, the nipple and areola are now centered higher on the chest wall. They have a youthful appearance to them, and are at the maximum point of projection. There is no longer a prolapsing of the breast tissue down towards the belly button. There is a nice contour of the breast from the upper chest wall to the peak of the nipple and the areola.
